This is a form designed to help preserve the everyday experiences of people living through the global pandemic of Coronavirus Disease (COVID-19). It involves writing a short diary entry and possibly uploading videos, photos, and other visual materials that you believe highlight the impact of COVID-19 on your daily life.

You should also change any other information that you believe could potentially identify you or close relations.
